Hospice Scales
Quantifying clinical assessments is necessary in hospice care through the use of scales and tools. Standardizing the manner in which we assess our patients and report our findings is easily achieved with the right scales and tools.
Regulators are increasingly evaluating hospice clinician documentation to find the evidence of clinical decline that quantifies the severity of illness, the severity of symptoms experienced and the extent of disease.
The hospice navigator series clinical guide “Hospice Scales Terms Tools and Measurements” provides easy access to the majority of assessment scales and tools that adequately and appropriately quantify the clinical findings and severity of symptoms. Clinical documentation systems often utilize these scales but hospice and palliative care organizations are best served by approving and implementing standardized tools to be used within the organization.
